Austin Crocker, PharmD
Lead Clinical Pharmacist. The current APOTHEcare biography states that Austin graduated from Samford University's McWhorter School of Pharmacy in 2017 and completed a residency with the University of Mississippi School of Pharmacy and Tyson Drugs in 2018, with training focused on caring for patients with chronic conditions.
APOTHEcare presents his work around collaboration with prescribers and services for Medicare populations, including Annual Wellness Visits, Chronic Care Management, and Remote Patient Monitoring. This demo does not add unverified licenses, certifications, outcomes, or ownership relationships.
